I already posted in the "Questions about Renshuu" thread, but after trying alternate methods, we think that this might be a bug. Thank you so much!

It's adding the correct terms, they just share the exact same hiragana spelling. You should see the difference if you tick "Study rare kanji" on all of them:
(this is from Reading Buddy, but you would have encountered the same issue if you'd added them from the dictionary)
You can turn on "Study rare kanji" directly from the dictionary:
Edit: You don't *need* it on all of them, just the version you'd like to study. I just did it as a demonstration.
